第五屆黃金時代之夜自然健美大賽策劃方案
賽事介紹：
黃金時代之夜健美大獎賽是由黃金時代科技健康産業有限公司董事長“高炎”先生于2003年創辦，至 2007年已經是該項賽事的第五次舉辦。此項賽事是目前國內健美大賽獎金及水平最高的健美賽事 ，此次大賽將會有來自世界各地衆多頂尖健美選手前來參賽。自2005年第三界黃金時代之夜健美大獎賽開始此次賽事已經備受位於美國的世界自然健美聯合會的重視，世界自然健美聯 合會主席“瑞克”先生在2005年的大賽時曾親臨賽場並對賽事給于了極高的評價並且也將此賽事列爲了2005年世界自然健美大賽中國區分站賽。並且今年“瑞克”先生也將親臨賽場，同時ESPN電視臺、河南電視臺、鄭州電視臺等國際國內電視臺也給予了賽事全面的報導。另外此次活動還有相關的健身産業交流展覽等相關行業的交流活動。

比賽級別安排： 男子健美：70公斤以下級（職業組）
男子健美：80公斤級（職業組）

男子健美： 男子健美：75公斤以下級（業餘組）
男子健美：85公斤級（業餘組）
男子健美：85公斤以上級（業餘組）
女子健身小姐：一個組別（無差別組）
專業組及業餘組參賽資格說明：（職業組比賽級別冠軍將親自受到世界自然健美聯合會主席瑞克邀請參加2007年11月在美國舉行的世界自然健美年終總決賽）、報名參加專業組比賽的選手須是曾經在全國健美大賽上獲得過前六名的選手，報名參加業餘組比賽的選手無特別條件要求，業餘組級別前2名自動升級爲下一年比賽職業組選手。如遇特殊情況選手最終資格認定需經大賽組委會研究決定。
比賽規定及規程：男子健美比賽分爲預、決賽、自由配樂表演（3分鐘以內）及全場總決賽前六名一對一PK賽。女子健身小姐組比賽分爲形體展示、規定路線行走及晚裝展示。
參賽運動員嚴禁使用國際奧會規定的禁用藥物，級別冠軍尿樣將送美國世界自然健美聯合會進行尿樣檢查。

我聲明本人身體狀況良好及具備足夠訓練參加該項賽事，且我僅聽從專業醫生的建議。
I certify that I am physically fit, have sufficiently trained for participation in the event and have not been advised otherwise by a qualified medical person.
我聲明該免責申明將用於本人所參加的所有賽程，且謹此豁免該賽事之承辦者、組織者、贊助商的責任。
I acknowledge that this Accident Waiver and Release of Liability form will be used by the event holders, sponsors and organizers, in which I may participate and that it will govern my actions and responsibilities at said event.
鑒於我申請並獲得該參賽資格，謹此本人，本人的執行人、管理人、繼承人、近親、接任人做出如下決定：(A) 免於追究黃金時代健身産業集團（Golden Times Fitness Club），環球健身公司（/Fitness Universe, Inc.），美國運動網路公司（/American Sports Network, Inc.）, ESPN傳媒（/ESPN, Inc.）, 環球城市製片公司（/Universal City Studios LLLP）,希爾頓通用公司（/Hilton Universal City）及其董事、成員、雇員、志願者、代表、代理，賽事的主辦方、贊助商對於本人因參加該項賽事而引起的包括死亡、殘疾、受傷、財産損失，或其他任何在賽程中可能發生在本人身上的傷害之責任；(B) 放棄向該段上述提及的組織及個人追究由於參加該項賽事所引起的任何賠償。
In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors,administrators,heirs,next of kin,successors,and assigns as follows:(A) Waive, Release and discharge from any and all liability for my death,disability,personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me including my traveling to and from this event, Golden Times Fitness Club, Fitness Universe, Inc. and American Sports Network,Inc.,ESPN,Inc.,Universal City Studios LLLP,Hilton Universal City, their directors, officers, employees, volunteers, representatives, and agents, the event holders, event sponsors, event directors, event volunteers;(B) Indemnify and Hold Harmless the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this event, whether caused by the negligence of releases or otherwise.
對本人在該賽事中發生的傷害、意外或疾病，本人同意接受治療。
I hereby consent to receive medical treatment, which may be deemed advisable in the event of injury, accident and or illness during this event.
該免責申明在適用法律下，應被詳細解釋用來提供最大限度的責任減免。
This Accident Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
考慮到本人之後可能有資格參加美國健身大會（/Fitness America Pageant） 和/或 美國比基尼小姐比賽（/Ms. Bikini America）和/或美洲錦標賽（/Model America Championships）和/或世界自然健美錦標賽（/Musclemania World Championships）和/或街舞錦標賽（/Hip hop Jazz Championships），故本人承認從該賽事中受益。我據此授權黃金時代健身産業集團（Golden Times Fitness Club）,環球健身公司（/Fitness Universe, Inc.），美國運動網路公司（/American Sports Network, Inc.）,其下屬組織和企業，傳媒和娛樂團體，環球城市製片公司（/Universal City Studios LLLP）及其所有代理、繼承者、許可人、指派人在無需本人審查而可使用或複製本人的照片（不論電影、磁帶、攝影、電影或其他）、聲音、形象、名稱用於展覽、散發、傳播。可以無時效的將本人的聲音、形象複製到不論是關於推銷、廣告或宣傳的任何包括攝影、照片、錄影、印刷品及任何現有媒介或將來可能出現的媒介中。且上述組織或個人有權使用本人的名字和資料。上述授權持續有效，上述組織或個人在任何時間都不必對本人做出賠償。黃金時代健身産業集團（Golden Times Fitness Club）,環球健身公司（/Fitness Universe, Inc.）,美國運動網路公司（/American Sports Network, Inc.）, 傳媒及娛樂團體及環球城市製片公司（/Universal City Studios LLLP）有義務使用該權利。這些權利是由黃金時代健身産業集團（Golden Times Fitness Club）,環球健身公司（/Fitness Universe, Inc.）,美國運動網路公司（/American Sports Network, Inc.）, 及環球城市製片公司（/Universal City Studios LLLP）獨家分配的。
And, in further consideration of permission being granted to me to participate in the Fitness America Pageant and /or Ms. Bikini America and/or Model America Championships and/or Musclemania World Championships and/or Hip hop Jazz Championships, their related event(the“Event”), receipt of which benefit is hereby acknowledge, I hereby grant you, Golden Times Fitness Club, Fitness Universe, Inc. and American Sports Network, Inc., and any and all Golden Times Fitness Club, Fitness Universe, Inc. and American Sports Network, Inc., approved video or entertainment organizations, as well as Universal City Studios LLLP, and all of their agents, successors, licenses and assigns, the right to photograph or otherwise reproduce(whether by film, tape, still photography, motion pictures or otherwise) my voice, appearance and name, and to exhibit, distribute, transmit, and/or exploit any and all such reproductions containing my voice and/or appearance in any and all media, including without limitation, by means of still photography, motion pictures, video, printing or any other media now known or hereafter devised, including also with respect to merchandising, advertising and/or publicity, and the right to use my name and information about me in any connection with the foregoing. The rights granted by me hereunder are granted for the entire universe and shall endure in perpetuity, and no further compensation shall be payable to me at any time in connection therewith. Nothing contained herein shall be deemed to obligate you, Golden Times Fitness Club, Fitness Universe, Inc. and American Sports Network, Inc., approved video or entertainment organizations, as well as Universal City Studios LLLP, to photography or otherwise reproduce my voice, appearance or name, or to make use of any of the rights granted herein. These rights my be assigned by Golden Times Fitness Club, Fitness Universe, Inc and American Sport Network, Inc., as well as Universal City Studios LLLP, at their sole and complete discretion.

I understand that you, Golden Times Fitness Club, Fitness Universe, Inc. and American Sports Network, Inc., approved video or entertainment organizations, as well as Universal City Studios LLLP, are videotaping and photographing the event in express reliance upon the foregoing, and I represent and agree that I am free to grant the rights granted to you, American Sports Network, Inc., Universal City Studios LLLP, and any and all approved video entertainment organizations hereunder.
