剛看到九月號的Clinical Journal of Sports Medicine報告了首例因努責效應 (Valsalva maneuver)導致的視網膜中央靜脈阻塞(CRVO),而患者正是老資格的 健美運動員,所以大家作重訓時真的要注意呼吸,尤其是重量做很重的大大們!

CASE REPORT A 37-year-old man presented with mild left-sided blurring of vision and metamorphopsia with duration of a few hours. His medical and ophthalmic histories were unremarkable. He was an experienced amateur bodybuilder and reported having set a personal record during the previous evening’s training, lifting about 105 kg (231.5 lb) of weight from the supine position (bench press)...
DISCUSSION The Valsalva maneuver, which is commonly applied by power athletes during resistance exercise and comprises the forcible exhalation against the closed glottis, has been associated with a host of systemic and ocular complications, including stroke, cerebral and subarachnoid hemorrhage, hiatal hernia, pneumothorax, and conjunctival and foveal hemorrhage. These events are thought to be secondary to the extreme pressure elevations that occur in the intra- abdominal, intrathoracic, intracranial, intraocular, and vascular compartments.
Valsalva maculopathy, a well-described, self-limited entity that usually resolves without compromising visual acuity, involves the spontaneous rupture of perifoveal capillaries and the occurrence of small intraretinal hemorrhages,
presumably due to the rapid rise of the retinal venous pressure following a corresponding intracranial pressure increase...
To the best of our knowledge, this is the first reported association of CRVO with the Valsalva maneuver, and both power athletes and trainers should be aware of this disturbing but probably benign condition 於健身健美推廣中心轉錄
